Role Overview

We are seeking a collaborative and skilled Therapeutic Case Manager to join our Behaviour and Specialist Programs Team. The successful candidate will deliver high level therapeutic case management, consultation to school teams and work proactively in the area of supporting learning and teaching.

Key Responsibilities:
  • Work within a multi-tiered system of support (MTSS) model.
  • Respond to referrals from school counsellor colleagues who request intensive support of an identified student and their family.
  • Manage a caseload of varying degrees of complexity.
  • Work within an intensive therapeutic case management model.
  • Collaborate with schools and other professionals to support them in meeting the needs of students presenting with complex and challenging behaviours.
  • Engage and work intensively with families to support their child and strengthen home-school partnerships across different schools.
  • Liaise with external agencies and professionals to enhance student support.
  • Support leaders and staff to respond effectively to student risk.
  • Consult and work with schools to construct tier 3 behaviour plans.
  • Collaborate with the school team and other stakeholders to conduct risk assessments and construct risk management plans to mitigate the risks presented by the student.
Requirements:
  • Demonstrate alignment to the core purpose and values of the organisation.
  • Have demonstrated understanding of the mental health needs of children and young people.
  • Experience in complex case management.
  • Experience in counselling families, children and young people using evidence-based interventions.
  • Experience in cognitive, emotional and behavioural assessment of children and young people.
  • Minimum of three years' experience as a registered Psychologist or Social Worker.
  • Have the capacity to make a significant contribution to matters related to student welfare and wellbeing.
  • Demonstrate evidence-based professionally informed judgement in dealing with sensitive issues.
  • Demonstrate the capacity to engage students, parents and teachers in the counselling process.
  • The capability to facilitate parent and caregiver education.
  • Be supportive of the organisations ethos.
