They will be able to work independently and as part of a team, and have a passion for delivering high-quality support services.

About the Role:

This role involves coordinating supports for individuals with disability, identifying and accessing necessary services and supports, and promoting inclusion and upholding customer's human rights. The successful applicant will also be responsible for maintaining knowledge of safeguarding standards and reporting requirements, and engaging in group practice reflection and decision making.

Key Responsibilities:

  • Coordinate, monitor and report on supports in accordance with customer plans;">
  • Support customers to meet their individual goals by identifying and accessing necessary services and supports;">
  • Maintain knowledge of safeguarding standards and reporting requirements;">
  • Promote inclusion and uphold customer's human rights;">
  • Engage in group practice reflection and decision making;">
  • Participate in team-based goal setting and monitoring;">
  • Service planning reviews and reporting to reflect internal and external customer deliverables and satisfaction;">

Required Skills and Qualifications:

The successful applicant will possess:

  • A formal qualification in social sciences, disability, community services or a related field (minimum Cert IV);">
  • Current driver's license (mandatory requirement);">
  • NDIS Worker Screening Clearance (or be willing to obtain).">
