Job Overview

The Disability Navigator role is a vital part of our team, focusing on providing care coordination and navigation for individuals with disabilities. This position plays a crucial role in ensuring successful hospital discharges by arranging appropriate support systems.

Our ideal candidate will possess strong clinical reasoning skills, experience working in the disability sector, and a deep understanding of navigating the National Disability Insurance Scheme (NDIS). They will be able to coordinate and communicate complex care plans within a healthcare context, as part of a multidisciplinary team. Additionally, they should have effective negotiation, advocacy, collaboration, and communication skills with various stakeholders.

Key Responsibilities
  • Provide care coordination and navigation services for individuals with disabilities
  • Develop and implement individualized care plans
  • Maintain accurate records and documentation
  • Collaborate with multidisciplinary teams to ensure comprehensive care

Required Skills and Qualifications
Tertiary qualification in Social Work or a relevant Allied Health profession
3 years' clinical experience as an Allied Health Clinician preferably in Social Work or a relevant Allied Health profession, with eligibility for registration with the appropriate professional body
