Responsibilities:

  • Coordinate care during the full continuum of cancer care experience.
  • Identify potential and real gaps in care delivery; seek out opportunities for avoidance and/or closure.
  • Organize and lead patient care conferences at regular intervals; communicate interventional outcomes to the care team in a timely manner.
  • Assess patient risk stratification.
  • Perform basic psychosocial assessments and provide emotional support; delegate referrals as needed.
  • Develop nursing diagnoses and action plans as needed.
  • Support oral chemotherapy education, adherence, and side effect monitoring.
  • Hold or participate in survivorship and advance care planning visits; make referrals as needed.
  • Participate in research, quality, improvement, including reporting on navigation metrics.
  • Support patient advocacy and empowerment.
  • Participate in community outreach and cancer prevention education.

Qualifications:

  • Bachelor of Science in Nursing required.
  • Five years of work experience as a Registered Nurse in an oncology practice or clinic.
