Job Title: Renal Care Coordinator

Central Australian Aboriginal Congress is seeking a skilled Renal Care Coordinator to join our team. As a Renal Care Coordinator, you will work collaboratively with Aboriginal clients and health professionals to provide culturally safe multidisciplinary and integrated care services for clients with complex chronic conditions of end-stage renal disease requiring haemodialysis.

Key Responsibilities:
  • Provide culturally safe care to Aboriginal clients with complex chronic conditions
  • Work collaboratively with health professionals to develop and implement care plans
  • Facilitate management of chronic disease(s) for clients receiving dialysis
  • Enhance the health and wellbeing of Aboriginal clients with significant chronic conditions
Requirements:
  • Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner
  • At least 2 years' post-graduate work experience
  • Excellent understanding of primary health care and sound clinical knowledge of chronic disease management
  • Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control
