We are seeking an experienced and motivated Clinical Care Coordinator to join our team at Royal Freemasons Ltd.
The successful candidate will provide clinical leadership and work collaboratively with the care team, residents, and families to oversee and guide effective resident-focused clinical outcomes.
Key Responsibilities- Undertake, or supervise others to undertake, clinical care for individual residents as required, including complex wound care, pain management, behaviour management, end-of-life care, and other clinical procedures as may be necessary.
- Liaise with relevant Residential care staff, medical, allied health, hospital, and other specialist health practitioners to ensure residents receive high-level clinical care.
- Focusing on clinical service delivery, compliance, and audit processes.
- Care planning in line with new standards.
