About the Role

We are seeking a highly skilled Clinical Care Specialist to join our team in the Hunter region. As a key member of our team, you will be responsible for providing high-quality clinical care and support services to our participants.

Key Responsibilities
  • Conduct comprehensive assessments and develop goal-oriented care plans for participants.
  • Evaluate and ensure the highest quality of care is provided to all participants.
  • Identify individual participant's needs and implement a clinical and/or general care plan and budget in consultation with the participant's carer and relevant stakeholders.
  • Manage workload effectively through excellent planning, organisational and time management skills.
About You
  • A registered nurse with current registration under the Australian Health Practitioner Regulation Agency (AHPRA).
  • Strong knowledge and practical experience working with people who are ageing from an enablement, restorative and wellbeing perspective.
  • A confident and skilled clinician, able to engage and motivate participants, families, staff and stakeholders.
