The Role

In the capacity of the CHSP Service Coordinator, you will oversee the efficient management of CHSP services for Amana Living's Home Care clients, their families, and caregivers. This position is offered as a Full-time contract, requiring 76 hours per fortnight, Monday to Friday from 8:30 AM to 4:30 PM.

Key Responsibilities:

  • Ensuring the coordination of services and client contact levels adhere to CHSP & Amana Living guidelines, while facilitating appropriate referrals for additional support services when necessary.
  • Serving as the initial point of contact for all CHSP referrals within the designated geographical area.
  • Assisting clients and their families in making informed choices regarding the care and services they receive.
  • Delivering safe and effective services that cater to the emotional, spiritual, and psychological well-being of clients.
  • Coordinating personalized and clinical care that aligns with best practices, tailored to individual needs to enhance health and well-being.
  • Conducting Health & Safety and environmental assessments for clients at the initial referral and annually thereafter.
  • Providing both telephone and face-to-face support to clients and caregivers within the assigned geographical areas.
  • Ensuring Community Support Workers adhere to the care plans developed by My Aged Care.
  • Timely maintenance and updating of all client case notes and files.
  • Actively working towards achieving and maintaining KPIs established for the designated geographical region.
  • Addressing client complaints and concerns proactively and professionally, investigating and resolving issues promptly.
  • Identifying, initiating, and coordinating quality improvement initiatives related to client care, including quality audits to ensure compliance.

About You

The ideal candidate will possess strong organizational and time management skills, demonstrating the ability to perform well under pressure and meet deadlines. You will have excellent interpersonal and communication skills, proficient computer skills, and the capability to quickly learn new software programs. You should be able to work independently as well as collaboratively as part of a team, acting as a change agent.

Essential Criteria:

  • Evidence of COVID-19 and current 2024 flu vaccinations in accordance with Amana Living policies.
  • Ability to obtain a National Police Clearance (valid within 6 months).
  • Successful completion of pre-employment forms and reference checks.
  • Valid 'C' Class driver's license and access to a personal vehicle.
  • Tertiary qualifications in Certificate IV in Aged Care and/or Community Care.
  • Previous experience in a Service Coordinator role or similar position.
  • Proficiency in Microsoft Office applications and Client Management databases.
  • Strong problem-solving and complaint resolution skills, with the ability to take initiative in decision-making.
