About the Role

Amana Living is seeking a highly skilled and dedicated CHSP Service Coordinator to join our team. As a key member of our Home Care team, you will play a vital role in ensuring the effective management of CHSP services to our clients, their families, and carers.

Key Responsibilities
  • Coordinate services and level of client contact in accordance with CHSP and Amana Living guidelines, and refer clients to additional support services as needed.
  • Act as the primary point of contact for all CHSP referrals within your designated geographical area, providing timely and effective support to clients and their families.
  • Support clients and their families in making informed decisions about their care and services, ensuring they receive the best possible support.
  • Provide safe and effective services that meet the emotional, spiritual, and psychological needs of clients, promoting their overall wellbeing.
  • Coordinate personal and clinical care that is tailored to individual needs, optimizing health and wellbeing outcomes.
  • Conduct regular health and safety checks, and environmental assessments to ensure a safe and supportive environment for clients.
  • Provide telephone and face-to-face support to clients and carers, responding to their needs and concerns in a timely and professional manner.
  • Ensure Community Support Workers work in accordance with My Aged Care care plans, providing high-quality support to clients.
  • Maintain and update client case notes and files in a timely and accurate manner, ensuring compliance with regulatory requirements.
  • Work proactively to achieve and maintain key performance indicators (KPIs) for your designated geographical region, identifying areas for improvement and implementing quality improvement activities.
  • Respond to client complaints and concerns in a professional and timely manner, investigating and resolving issues to the client's satisfaction.
About You

We are seeking a highly organized and motivated individual with excellent interpersonal and communication skills. You will have a proven ability to work well under pressure, meet deadlines, and prioritize tasks effectively. A strong understanding of the aged care sector and experience in a similar role would be highly regarded.

Essential Criteria
  • Certificate IV in Aged Care and/or Community Care
  • Previous experience as a Service Coordinator or similar role
  • Valid 'C' Class Drivers Licence and personal vehicle
  • Ability to obtain a National Police Clearance
  • Successful completion of pre-employment form and reference check
