Job Overview:

We are seeking an experienced and compassionate aged care professional to join our team as a Clinical Assessor. In this role, you will conduct comprehensive assessments to determine the needs of our customers who require home support services.

As a key member of our assessment team, you will play a vital role in developing personalized support plans that meet the unique needs of our customers. Your expertise will help us provide high-quality care and support to those who need it most.

Key Responsibilities:
  • Collaborate with customers, carers, families, representatives, and other service providers to assess their needs and develop personalized support plans.
  • Undertake and review assessments, managing changing customer and carer needs with consent.
  • Identify customers' formal and/or informal respite support and arrangements.
  • Develop Emergency Care Plans if the customer does not have one in place.
  • Refer for services, assess complexity, and risk of vulnerability, and assist with linking support, reablement, and short-term case management.
  • Participate in clinical supervision and undertake various administrative functions, including report writing and correspondence preparation.
Requirements:
  • Tertiary qualification in a health-related discipline (e.g. Registered Nurse, Physiotherapist or Social Worker).
  • Current unrestricted registration or part of, or eligible to be part of, a relevant professional association.
  • Minimum of one year demonstrated experience delivering services in aged care settings and/or to aged persons.
  • Knowledge or experience in conducting home assessments or risk assessments for vulnerable people.
  • Demonstrated ability to communicate clearly and effectively with customers with hearing loss, sensory loss, or cognitive impairment.
  • Experience in undertaking assessments of customers with dementia who have responsive behaviours.
  • Adaptability and flexibility to respond to changing task requirements in a positive way.
