This is a permanent full-time opportunity for an experienced Registered Nurse to work as a Transitional Nurse Practitioner in palliative care at a leading healthcare organisation.
About the Role:- The Transitional Nurse Practitioner will provide expert clinical support to the Community Palliative Care Team, with a focus on palliative care and aged care assessment skills.
- The role involves providing support for educational and professional development of nursing groups, palliative care staff, patients, carers, and their families.
The core function of this role is to act as a conduit for residents of aged care facilities or other hospitals transitioning to end-of-life care, establishing links with appropriate clinical services.
About You:- You must be a Registered Nurse willing to enrol in a Tertiary Master's program for Nurse Practitioner.
- You need to have a minimum of 5000 hours of advanced nursing/midwifery practice during the last six years, appropriate to the specialty area.
- Effective communication and interpersonal skills, organisation, networking, and time management skills are required.
