Job Description:

We are seeking an experienced Clinical Care Manager to oversee the daily operations of our in-home aged care and NDIS services, ensuring that our clients receive the highest standard of care.

  • Manage and coordinate the delivery of in-home aged care and NDIS services
  • Supervise and support a team of care professionals
  • Develop and implement care plans tailored to individual client needs
  • Ensure compliance with all relevant regulations and standards
  • Liaise with clients, families, and healthcare professionals to ensure seamless care delivery
  • Monitor and evaluate the quality of care provided, making improvements as necessary
  • Conduct comprehensive assessments and investigate issues and incidents
  • Ensure adherence to Right at Home's best practices and standards
  • Supervise, train, and mentor Registered Nurses (RNs) and Assistants in Nursing (AINs)
  • Provide support to the Care Management Team

Requirements:

  1. AHPRA Registration
  2. Minimum of 3 years of experience in a similar role
  3. Must be a Registered Nurse with a strong clinical background
  4. Aged Care and NDIS experience is preferred
  5. NDIS Workers Screening Card
  6. Demonstrated working knowledge of current aged care standards, accreditation, and auditing processes
  7. Exceptional interpersonal skills, extensive leadership, and management experience
  8. Current Flu and Covid Vaccinations
  9. A valid Police Check
  10. Full working rights in Australia
  11. Driver's Licence
  12. First Aid and CPR Certificate
  13. Excellent computer skills (Word, Excel, and Outlook)
Key Responsibilities

The successful candidate will be responsible for overseeing the daily operations of our in-home aged care and NDIS services. This includes managing and coordinating the delivery of these services, supervising and supporting a team of care professionals, developing and implementing care plans tailored to individual client needs, and ensuring compliance with all relevant regulations and standards.

The ideal candidate will have excellent communication and interpersonal skills, as well as a strong clinical background and experience working in aged care and NDIS settings.

Requirements

To be considered for this role, you must possess the following qualifications and experience:

  • AHPRA registration
  • Minimum of 3 years of experience in a similar role
  • Registered nurse with a strong clinical background
