Job Overview
The Family Housing Support Outreach Case Manager plays a vital role in delivering direct service to families who are homeless or at risk of homelessness. The primary objective is to secure long-term affordable housing and provide sustainable support for these families.
Key Responsibilities:
- Assist families in obtaining and maintaining stable, long-term accommodation following their exit from crisis housing or referral.
- Provide case management services to address the specific needs of each family and establish ongoing support referrals.
- Rapidly respond to immediate housing and support requirements; facilitate the delivery of Transitional Housing Management (THM) properties.
- Increase awareness among families regarding available services and supports, with a focus on preventing tenancy failure.
- Educate families on their rights and responsibilities under the Residential Tenancies Act 1997.
- Promote overall wellbeing, self-efficacy, and timely access to assistance.
- Ensure that services align with the National Homelessness Stream Model of Care.
Required Qualifications and Skills:
- Tertiary qualifications in Social Work or a related field within community services.
- Demonstrated knowledge of homelessness, its context, legislation, models, and practice.
- Experience working with culturally and linguistically diverse communities.
- Strong assessment, case management, and collaboration skills.
