About this Role

As a Chronic Disease Management Professional, you will be responsible for helping patients manage their chronic conditions.

Key Responsibilities:
  • Develop and implement care plans to help patients effectively manage their disease/s
  • Empower patients to take ownership of their health and well-being
  • Coach patients on healthy behavior change principles to improve long-term health outcomes
  • Facilitate access to ongoing care from a multi-disciplinary team to ensure comprehensive support
  • Conduct regular health assessments to identify early warning signs of chronic disease
Required Skills and Qualifications:
  • AHPRA registration as an Enrolled or Registered Nurse
  • Proven experience in Chronic Disease Management and health assessments
  • Demonstrated knowledge of Health Assessment is highly regarded
