Expert Patient Advocate

This position plays a significant clinical role through the provision of expert clinical assessment, referral and coordination of health and community services for individuals with severe chronic disease and complex care requirements living in the community who are at risk of deteriorating health that may result in declining quality of life and an acute hospital admission.
The ideal candidate will have strong communication and interpersonal skills, with the ability to work effectively in a dynamic team environment.
Key Responsibilities:- Provide expert clinical advice to patients, carers and other healthcare professionals within South Eastern Sydney Local Health District, community organisations, primary care/General Practice, Non-Government Organisations (NGOs) and other government departments to support integration of care.
- Coordinate and navigate across the healthcare system and connect with other community and hospital-based coordination services as required in drug and alcohol, mental health and primary care.
- Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia.
- Minimum five years full-time equivalent postgraduate qualification experience and a postgraduate qualification in a relevant field of practice, or such other qualifications or experience deemed appropriate.
- Demonstrated experience working with and extensive knowledge of the broader health and social issues that people with chronic and complex conditions require.
