Job Title: Clinical Nurse Specialist

This is an exciting opportunity to join our multidisciplinary team as a Clinical Nurse Specialist. You will work closely with healthcare professionals to provide high-quality care to patients with chronic diseases.

Key Responsibilities:
  • Navigate care of patients and facilitate services to manage their health in a way that will result in optimal health outcomes.
  • Provide care in a range of modes and settings, including home visits, multidisciplinary clinics, virtual care, and group classes.
  • Collaborate with external stakeholders, including primary health networks, NDIS, My Age Care, other local health districts, and non-government organizations.
Required Skills and Qualifications:
  • Current registration as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
  • Relevant post-registration qualifications and at least three years' experience working in the clinical area of their specified post-graduate qualification.
  • Demonstrated effective time management skills and ability to prioritize competing workloads within a complex environment.
  • Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking, and analysis in daily practice as a registered nurse.
