COVID-19 Pathways Operations Lead

We are seeking a skilled professional to lead the COVID-19 Positive Pathways model of care for the South East Metro Health Service Partnership.

  • The ideal candidate will have experience in high-level partnerships and engagement with key stakeholders to promote positive relationships and outcomes for clients.

This is a full-time, 12-month fixed-term position that requires strong collaborative professional leadership, teamwork, and outstanding communication skills to achieve positive client outcomes.

The role involves overseeing strategic and operational coordination of COVID-19 governance activities for the South East Metro Region. This includes working with contracted Community Health Services, Primary Care, and Public Health Partners.

The successful applicant will be instrumental in supporting the transition of the COVID-19 Positive Pathways model of care and will play a key role in ongoing strategy and coordination within the Health Service Partnership.

Key Responsibilities:

  • Developing and implementing strategies to improve patient outcomes and quality care.
  • Building and maintaining effective relationships with internal and external stakeholders.
