Compassionate Community Care Professional

A caring and compassionate individual is required to join our Community Care Team. The successful applicant will provide person-centred care in the community, ensuring a safe environment and applying clinical skills to improve quality of life through client-focused activities.
The role involves building meaningful relationships with clients and contributing to the team's success. You will work collaboratively with multi-disciplinary team members to assess, plan and implement client care within your scope.
- Consider the physical, social and psychological requirements of clients.
- Create and update care plans to reflect developing needs.
- Ensure clinical quality compliance standards are met.
The job requires on-the-road community visits and scheduled client visits most days.
About This Role- AHPRA registration and unlimited work entitlements.
- Post-registration general nursing experience.
- Excellent communication and clinical problem-solving skills.
- Australian Driver's Licence (Green Provisional minimum).
- A roadworthy and insured vehicle to use for work.
