Job Title: Compassionate Care Professional

About the Role:
  • We are seeking a dedicated individual who wants to build relationships and provide quality care to our clients.
  • You will conduct initial client intake and screening assessments, develop and review individualized care plans, and provide support with daily activities.
Key Responsibilities:
  1. Conduct thorough initial screening assessments and develop care plans for personal self-care, general house cleaning, social support, respite, transport, and shopping assistance.
  2. Manage client information in CRM & CMS, ensuring all details and case notes are up-to-date.
  3. Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team.
  4. Provide care in accordance with individualized care plans and support with community-based recreational and social activities.
  5. Transportation and assistance with shopping and appointments.
Requirements:
  • Certificate III Aged Care or an equivalent qualification.
  • Australian Driver's Licence (minimum provisional).
  • Roadworthy and insured vehicle to use for work.
  • Experience with technology, including smartphones, tablets, and laptops.
  • Confidence to satisfy relevant probity checks required by legislation or policies.
