Job Summary:

We are seeking a compassionate and empathetic Care Partner to join our care team.

Key Responsibilities:
  • Partner with clients to develop and review personalized care plans that meet their unique needs and goals.
  • Build trusting relationships with clients, families, and providers by navigating complex issues and sensitive situations with professionalism and care.
  • Provide clear education around service options, helping clients access more services and get the best value from their package.
  • Proactively identify risks and opportunities to enhance client outcomes, ensuring safe, high-quality care supported by accurate documentation.
About You:
  • You will need customer service or call centre experience, preferably in case management, client support, or account management.
  • You must have strong communication skills, including the ability to educate and guide clients through complex conversations.
  • You should be proactive in uncovering opportunities that help clients make the most of their service package.
  • You will work effectively under pressure, maintaining a calm and solutions-focused mindset when challenges arise.
