About the Role

You will serve as a vital link between our clients, their families, and our care team, ensuring seamless delivery of home care services through meticulous schedule management, precise care planning, and empathetic support.

Main Responsibilities:
  • Mate clients with caregivers based on needs and preferences
  • Schedule and coordinate in-home care services
  • Foster strong relationships with families, caregivers, and healthcare professionals
  • Monitor and update client care plans to guarantee optimal outcomes
  • Ensure compliance with NDIS standards and aged care best practices
  • Deliver outstanding customer service and support to clients
Requirements:
  • 2 years experience in aged care/disability care and knowledge of NDIS standards and Aged Care Standards preferred
  • Strong organizational skills
  • Ability to multitask and work effectively in a fast-paced environment
  • A compassionate attitude
  • Proficiency with scheduling software and basic computer skills
