Job Description

We are seeking a dedicated Care Coordinator Specialist to join our home care support team. The ideal candidate will be responsible for ensuring that our clients receive the highest quality home care services by managing schedules, coordinating care plans, and providing compassionate support.


About the Role:

This is a full-time position that requires strong organizational and communication skills. As a Care Coordinator Specialist, you will be the key link between our clients, their families, and our care team. You will work closely with caregivers, healthcare professionals, and family members to ensure that our clients receive the best possible care.


Key Responsibilities:
  • Match clients with caregivers based on needs and preferences
  • Schedule and coordinate in-home care services
  • Communicate with families, caregivers, and healthcare professionals
  • Monitor and update client care plans
  • E nsure compliance with relevant standards and best practices

Requirements:

To be successful in this role, you will need 2 years of experience in aged care/disability care and knowledge of relevant standards and best practices. You should also have strong organizational and communication skills, as well as the ability to multitask and work in a fast-paced environment.
