Senior Community Nurse Role

This is a meaningful opportunity to make a real difference in the lives of older people—helping them stay well, stay safe, and stay connected.
- Key Responsibilities:
- Provide direct clinical nursing care to clients in their homes in line with best practice nursing procedures.
- Conduct clinical assessments, dressings, medication assessment, and catheter changing.
- Develop a holistic nursing care plan in collaboration with client, carer, Care Advisor, and Allied health where relevant.
- Provide safe, quality nursing practice to clients with specific needs and/or complex needs, such as wound care, palliative care, education, and support.
- Liaise with all parties involved in the care of the client to maintain holistic healthcare and wellbeing.
- Ensure all documentation is accurate and completed in a professional and timely manner.
- Work in a safe manner to protect yourself, your colleagues, and clients, closely following all policies and procedures.
- Requirements:
- A genuine passion for providing high-quality care to the elderly.
- Current AHPRA registration following completion of a Bachelor of Nursing or similar.
- Minimum 3 years postgraduate nursing experience.
- Experience in clinical assessments.
- Initiative and the ability to work independently.
- Strong computer skills, especially Microsoft Office products, and the ability to pick up new systems.
- NSW Driver's license and a reliable vehicle.
