Job Description:
  • Provide high-quality, direct clinical nursing care to clients in their homes in line with best practice nursing procedures and guidelines.
  • Conduct comprehensive clinical assessments, dressings, medication reviews, and catheter changes as required.
  • Develop individualized nursing care plans that address the unique needs and goals of each client in collaboration with healthcare professionals and family members.
  • Liaise with various stakeholders to ensure holistic healthcare and well-being for clients with complex needs or specific requirements.
  • Maintain accurate and up-to-date documentation of client care and progress in a professional and timely manner.
Key Requirements:
  • A genuine passion for delivering exceptional care to seniors and elderly individuals.
  • Current registration with the relevant nursing authority (e.g., AHPRA) following completion of a Bachelor of Nursing or equivalent qualification.
  • A minimum of three years of postgraduate nursing experience, preferably in a community or home-based setting.
  • Strong assessment and clinical skills, including experience with wound care, palliative care, and patient education.
  • The ability to work independently and take initiative, while also being part of a collaborative team.
  • Excellent communication and interpersonal skills, with a focus on empathy and understanding.
  • Proficiency in Microsoft Office applications and other relevant software systems.
  • A valid driver's license and access to a reliable vehicle for travel between clients' homes.
