Home and Community Care Nurse

This is a challenging yet rewarding role that involves providing high-quality care to older adults living independently in the community.

About the Role
  • This position requires a skilled nurse with experience in clinical assessments, wound care, palliative care, and education and support.
  • The successful candidate will be able to work independently and as part of a multidisciplinary team to provide holistic, person-centred care to clients with specific needs and/or complex needs.
Responsibilities
  1. Providing direct clinical nursing care to clients in their homes in line with best practice nursing procedures.
  2. Conducting clinical assessments, dressings, medication assessment and catheter changing.
  3. Developing a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant.
  4. Liaising with all parties involved in the care of the client to maintain holistic health care and wellbeing.
  5. Ensuring all documentation is accurate and completed in a professional and timely manner.
Requirements
  • A genuine passion for providing high-quality care to the elderly.
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar.
  • Minimum 3 years postgraduate nursing experience.
  • Experience in clinical assessments.
  • Initiative and the ability to work independently.
  • Strong computer skills especially Microsoft Office products and ability to pick up new systems.
