We are seeking an experienced clinician to join our team as a Care Coordinator in our Virtual Care Centre. As a Care Coordinator, you will play a critical role in managing acute patient care, enhancing and promoting their quality of life. This is a busy service that demands the best talent to help us deliver high-quality care to our patients.

About the Role

This is a casual position that will be offered on a fixed-term basis until the end of September 2025. You will work across a 7-day roster between the hours of 7am and 10pm, providing coverage for leave and assisting with periods of high service demand. You will be based in our Virtual Care Centre in Eastwood, Adelaide, and will have the option to work from home or our office.

Key Responsibilities

As a Care Coordinator, you will be responsible for managing patient's acute care health requirements in a professional manner. You will add value by establishing, monitoring and coordinating patients' treatment plans, utilising virtual technology and in partnership with our home visiting provider team. You will require proficient computer skills to enable fast and accurate data entry into patient electronic medical records.

Requirements

We are looking for an experienced clinician in nursing (RN) with acute assessment skills, care planning, coordination skills and intuitive people skills. You will be knowledgeable about acute healthcare practices and bring strong clinical reasoning and assessment skills. Recent experience in acute care management in a hospital or community setting amongst a multidisciplinary team will be highly regarded.
