The Opportunity

As a skilled Clinical Care Manager, you will leverage your expertise in acute patient care to enhance and promote the quality of life for our patients. This is a dynamic role that demands exceptional clinical, communication, and problem-solving skills to lead and deliver high-quality care.

Our team operates on a 7-day roster between 7am and 10pm, offering both full-time and part-time opportunities. This position is based in our Virtual Care Centre (VCC) in Eastwood, Adelaide, with flexible working options for employees who prefer to work from home or our office.

As a key member of our multidisciplinary team, you will be responsible for managing patient health requirements, establishing and coordinating treatment plans, and utilizing virtual technology to deliver best practice care. Your acute assessment skills, care planning, and coordination skills will be essential in ensuring our patients receive high-quality care.

This role is a fixed-term position until the end of September 2025.

About You

As an experienced clinician in nursing (RN), you will bring a strong understanding of acute healthcare practices and possess excellent clinical reasoning and assessment skills. You will be knowledgeable about care coordination and have proficient computer skills to enable fast and accurate data entry into patient electronic medical records.

We are a fast-paced service operating in a virtual environment, and as a Clinical Care Manager, you will require strong communication and interpersonal skills to work effectively with our multidisciplinary team. Recent experience in acute care management in a hospital or community setting will be highly regarded.
