Catholic Healthcare is seeking an experienced Clinical Aged Care Assessor to join our dynamic team. As a key member of our Aged Care Assessment Services (ACAS) team, you will play a pivotal role in assessing the complex needs of clients and referring them to appropriate aged care services.

About the Role

This is a fantastic opportunity for a skilled healthcare professional to work with our multidisciplinary teams to promote a supportive and positive culture. You will be conducting accurate assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services. Your day-to-day responsibilities will include collaborating with clients, families, and multidisciplinary teams as required, reviewing support plans to meet client's changing support needs, and working effectively both autonomously and as a member of a broader team to deliver to key performance indicators within identified timeframes.

About You

To succeed in this role, you will have a tertiary qualification in a health-related discipline directly related to Health, Aged Care, or a related specialist area. You will also have current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) and an Australian Driver's license and access to a registered, insured vehicle. A minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons, including people living with dementia, is also essential.

Catholic Healthcare offers a competitive salary package, estimated at $54.47 per hour plus superannuation, paid parental leave, and long service leave after 5 years. You will also have access to fitness passport discounted gym memberships, travel reimbursement at 0.88 cents per km, and flexible hours and days to suit your work-life balance.
