Job Title: Outreach Support Worker

Carbal Medical Services is a Not-For-Profit organisation that provides health services to the Aboriginal and Torres Strait Islander communities in and around Toowoomba and Warwick.

Job Overview

We are seeking an experienced candidate to provide exceptional care and support for some of our most vulnerable people in their homes and in the community.

Key Responsibilities
  • Work one-on-one with community members in gaining access to the supports they need;
  • Be driven, motivated and have a passion to assist community members to foster independence and resilience and achieve their goals;
  • Work closely with GPs, Aboriginal Health Workers, and other health professionals;
  • Build and maintain respectful and meaningful community relationships.

Requirements
  • You will have experience in working effectively with Aboriginal and Torres Strait Islander people and a clear understanding of Aboriginal and Torres Strait Islander culture;
  • You will have a demonstrated understanding of the health and social issues found in our Indigenous communities;
  • You must have an Open C Class driver license, possess a Blue Card, and be willing to undergo a Police Check;
  • You will have a demonstrated ability to work as part of a team;
  • A Mental Health and/or Aboriginal Health Worker qualification would be highly desirable.
