Case Manager Position

This role is for a professional who will work collaboratively with families experiencing complex issues, providing client-focused and outcome-based services. The primary responsibility is to deliver tailored support to reduce risks and create positive change within home environments.

As a Case Manager, you will be crucial in fostering respectful relationships with stakeholders, government departments, and the community to ensure seamless service delivery. You will also advocate for the needs of Aboriginal people, contribute to cultural case plans, and commit to trauma-informed care under the Sanctuary model.

Key Responsibilities:
  • Advocate for the needs of Aboriginal people, contributing to cultural case plans.
  • Commit to trauma-informed care under the Sanctuary model, integrating it into your daily work.
  • Foster respectful relationships with stakeholders, government departments, and the community for seamless service delivery.
  • Develop and implement case plans in collaboration with families and other service providers.
Required Qualifications and Skills:
  • Relevant qualifications in the Human Services field (Diploma of Community Services or equivalent).
  • Practical experience in working with vulnerable families and an ability to identify children and young people at risk.
  • Demonstrated expertise in client-centred case management.
  • Knowledge of the Community Services sector, trauma and its impacts, crisis impacts on families, child protection issues, and legislation.

The ideal candidate will have excellent communication and interpersonal skills, be able to work independently as part of a broader dedicated team, and have a passion for delivering high-quality services that make a real difference in people's lives.

Applicants must obtain a Working with Children Clearance through the Office of the Children's Guardian NSW, and a satisfactory National Criminal History Check, which HR will undertake.
