Case Manager Position Overview

This position outlines the key responsibilities and requirements for the role of a Case Manager in our Family Capacity Building Program.


Job Description

As a skilled Case Manager, you will be working collaboratively with clients experiencing complex issues to deliver client-focused and outcome-based services. Your primary responsibility is to work independently as part of a broader dedicated team, connected daily via communication platforms and phone. You will support families in meeting their needs, with a focus on cultural inclusivity and trauma-informed care, to improve the lives of vulnerable children.


Key Responsibilities:
  • Advocate for the needs of diverse individuals, contributing to culturally sensitive case plans.
  • Commit to trauma-informed care under the Sanctuary model, integrating it into your daily work.
  • Foster respectful relationships with stakeholders, government departments, and the community for seamless service delivery.
  • Develop and implement case plans in collaboration with families and other service providers.
  • Ensure that the voices of clients and families are heard in case planning and goal setting.
  • Work respectfully to ensure that case plan goals are achievable and realistic.
  • Support and engage parents and children attending playgroup activities.

Required Skills and Qualifications:
  • Relevant qualifications in the Human Services field (Diploma of Community Services or equivalent; Degree qualifications preferred).
  • Practical experience in working with vulnerable families and an ability to identify children and young people at risk.
  • Demonstrated expertise in client-centred case management.
  • Knowledge of the Community Services sector, trauma and its impacts, crisis impacts on families, child protection issues, and legislation.

  • Salary range from $44.58 to $47.97 per hour, plus 12% superannuation.
