About the Role:

This role is focused on supporting clients with chronic conditions.

  • The goal is to provide comprehensive care that meets the needs of our clients.

You will work in a dynamic and growing healthcare network, collaborating with other services within Clinical Divisions as well as local Aboriginal Health Services.

Key Responsibilities:

  • Collaborate with clients to set realistic goals and develop personalized care plans.
  • Provide ongoing education and support to promote self-management strategies.
  • Work effectively within an interdisciplinary team to deliver high-quality patient care.
  • Evaluate and adjust therapy techniques to ensure optimal outcomes for each client.

Requirements:

  • Possess knowledge and experience working with clients who have chronic conditions.
  • Have a passion for helping others achieve their health and wellness goals.
  • Demonstrate strong communication skills and ability to build trust and rapport with clients.
  • A desirable skillset includes behaviour change, health coaching or chronic disease management and experience with evidence-based frameworks.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Empathy and patience are essential in delivering client-centred care.

Qualifications:

  • An appropriate degree or equivalent qualification for one of the following Allied Health disciplines: Dietetics, Exercise Physiology, Occupational Therapy, Physiotherapy, Podiatry or Psychology.
