Job DescriptionWe seek an energetic and personable Liaison to connect individuals and families with the care they deserve.
As our representative in the field, you will build trusted relationships with referral partners, generate new client opportunities, and contribute to our mission of personalized home care.
The key to success lies in developing and nurturing relationships with referral sources such as hospitals, physician offices, skilled nursing facilities, senior living communities, social workers, and discharge planners.
- Develop and maintain strong relationships with healthcare professionals.
- Promote our services through consistent outreach and presentations.
- Identify and convert leads into active clients.
- Partner with our care coordination team to ensure a smooth onboarding experience.
- Meet sales targets aligned with agency goals.
- Track referral outcomes and follow-up activity.
- Represent us at local events and health fairs to build brand awareness.
- Stay informed about industry trends and competitor offerings.
- Assist with administrative tasks as needed.
- Contribute positively to a collaborative team environment.
