Community Health Outreach Specialist

The Community Health Outreach Specialist will be a vital member of a multidisciplinary team, delivering and developing health provision to marginalised and vulnerable adults in the community.
Key Responsibilities- To manage client cases, assess need and risk, and deliver outreach services to hard-to-reach groups.
- To provide a flexible, accessible, and non-stigmatising service to marginalised and vulnerable adults who struggle to engage with mainstream health and social care services.
- To work within an allocated MDT, carrying out clinical duties assigned by the Clinical Team Manager.
- To lead in supporting and helping service users become aware of their own physical health and wellbeing, and engage them in addressing these needs.
- Registered Nurses where required, subject to satisfactory completion of competency assessments.
- Other professionally registered staff, subject to satisfactory competency assessment, taking responsibility for medication management for service users.
- To follow trust policy in promptly reporting all accidents and incidents.
- 27 days holiday, plus bank holidays, rising to 33 days after 10 years service.
