Aged Care Clinical Coordinator Role

Royal Freemasons is seeking an experienced Clinical Care Coordinator to join our team at Footscray Residential Aged Care Facility.
About the Role- This position involves providing clinical leadership and working collaboratively with the care team, residents, and families to oversee effective resident-focused clinical outcomes.
The successful candidate will be a Registered Nurse with experience in caring for high care and dementia residents, and have knowledge of wound care, palliative care, behavior management, clinical assessments, and care plans.
Responsibilities- Undertake or supervise others to undertake clinical care for individual residents as required, including complex wound care, pain management, behavior management, end-of-life care, and other clinical procedures.
- Liaise with relevant residential care staff, medical, allied health, hospital, and other specialist health practitioners to ensure residents receive high-level clinical care.
- Focusing on clinical service delivery, compliance, and audit processes, as well as care planning in line with new standards.

- A current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA).
- Recent experience in caring for high care and dementia residents.
- Knowledge of wound care, palliative care, behavior management, clinical assessments, and care plans.
- A valid Police Clearance or National Disability Insurance Scheme (NDIS) Worker Screening Clearance, or potentially both, to undertake the inherent requirements of this role.
- All aged care workers must be vaccinated against Covid-19 (3 vaccines) and are encouraged to receive the Flu-Vaccine, with proof of Covid-19 vaccination required prior to commencement.
