Clinical Specialist Palliative Care Lead

We are currently seeking an experienced Clinical Specialist Palliative Care Lead to join our dedicated team and contribute to the delivery of exceptional end-of-life care across our eight aged care homes.
The Role
You will report directly to the Executive Manager-Clinical Governance and be responsible for developing and implementing innovative service models, ensuring the delivery of outstanding palliative care. You will assist in providing specialist palliative care and end-of-life care to older persons, residents/participants, families, and other stakeholders facing palliative care needs.
Key Responsibilities
- Maintain a person-centred approach in assessment, planning, delivery, and evaluation of older persons' care and service provision
- Deliver clinical support tailored to individual needs and take a leading role in developing palliative care and end-of-life care plans
- Provide continuity of care to ensure effective palliative care and end-of-life care
- Identify opportunities for developing palliative care and end-of-life care nursing services to improve older persons' care
- Manage therapeutic interventions and referrals as required, along with strong palliative care and end-of-life care pharmacology knowledge
- In consultation with residents, participants, families, CCCs, and GPs, coordinate review and revision of care plans for palliative care and end-of-life care
- Promote collaborative working practices in all aspects of palliative care and end-of-life care
- Deliver In-Service Education Programs and Toolbox Talks to relevant personnel, residents, participants, and families
- Support and provide education and knowledge transfer about palliative care and/or end-of-life care to residents/participants, their families, and staff
- Participate in case conferences and monthly care statements for residents/participants regarding palliative care and/or end-of-life care
- Monitor and evaluate care provided and provide ongoing assessment of resident/participant health status in accordance with defined policy and procedures
Requirements
- Dedication to resident care and support
- At least five years' experience as a Registered Nurse (Div 1)
- Strong knowledge and experience working with palliative care and end-of-life care technology and education
- Developing palliative care and end-of-life care plans
- Ability to interpret aged care and disability standards and develop or implement related policies and procedures
- Excellent interpersonal, written, and verbal communication skills
- Ability to effectively communicate with residents, resident representatives, and other health professionals
- A good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to clinical care
