Job Description

We are seeking a dedicated and compassionate professional to join our team as a Clinical Care Partner. In this role, you will have the opportunity to make a positive difference in the lives of older people and individuals living with disability.

Your key responsibilities will include supporting aged care and NDIS participants through assessment, planning, coordination, and ongoing review of care. You will ensure safe, high-quality, person-centred services that reflect everyone's clinical, social, emotional, and cultural needs.

To achieve this, you will:

  • Complete holistic assessments and develop care plans tailored to each participant's unique needs.
  • Coordinate services and support teams to deliver care aligned with participant goals and objectives.
  • Maintain accurate documentation and meet program and compliance requirements.

Required Skills and Qualifications

To be successful in this role, you will need:

  • A caring and friendly approach, with excellent communication skills.
  • Proven relationship management skills and good interpersonal, negotiation, and problem-solving skills.
  • Able to work as part of a team and contribute to a collaborative environment.

Qualifications

You will require:

  • Qualification in a health or allied health profession (e.g. Social Worker, Registered Nurse, Occupational Therapist).
  • Experience in care planning, case management, or service coordination.
  • Strong understanding of aged care, NDIS, and community health systems.
