Clinical Case Coordinator
Description:
The Clinical Case Coordinator will play a pivotal role in the collection of background information about clients, liaison with key stakeholders and managing administrative tasks. The service aims to provide children and young people with neurodevelopmental vulnerability access to assessment, treatment, management and support through a multidisciplinary team.
This position requires a minimum of diploma qualified or relevant experience in a clinical case coordination role or similar. Demonstrated experience and skills in providing clinical case management support to vulnerable families that is child focussed, family centred and trauma-informed are essential.
Key Responsibilities:
- Collecting and analysing client data for assessment purposes.
- Liaising with internal and external stakeholders to ensure seamless services delivery.
- Managing administrative tasks and coordinating resources as required.
Required Skills and Qualifications:
- Diploma qualification or relevant experience in clinical case coordination.
- Experience and skills in providing clinical case management support to vulnerable families.
- Knowledge of child development and neurodevelopmental disorders including FASD, ASD and Global Development Disorders.
