Clinical Preventive Care Specialist

We are seeking a skilled Nurse Clinical Specialist to provide specialist clinical outreach care to remote clients and support PPHC staff in improving outcomes for preventable chronic conditions.
About the RoleThe successful candidate will be accountable for providing sound professional advice, assessment, planning, review, evaluation, and coordination of comprehensive client care and/or case management. This will involve working with clients to improve their health outcomes and provide education on preventative measures.
Responsibilities- Provide specialist clinical outreach care to remote clients.
- Support PPHC staff in improving outcomes for preventable chronic conditions.
- Assess, plan, review, evaluate, and coordinate comprehensive client care and/or case management.
- Develop and implement effective strategies to improve health outcomes for clients.
- Current registration with Nursing and Midwifery Board of Australia or eligible for same.
- High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills.
- Ability to travel to remote communities as required.
- Hold a current NT driver's licence (or eligible to obtain) and be confident driving a manual vehicle in regional and remote settings.
