Case Manager

The Case Manager is a vital member of the Transition Care Program (TCP) multidisciplinary team at Eastern Health. This role is crucial in the management of clients in our Transition Care Program, which operates at three residential sites within the Eastern Health catchment and via a community-based service for clients in their own homes.
The Case Manager works collaboratively with clients, their families/carers, and other members of the multidisciplinary team to facilitate the client's transition back into the community or into a residential care facility of their choice. They facilitate collaboration between various team-based multidisciplinary groups and coordinate the provision of services. The Case Manager also assists clients and their family/carers with adjusting to changes in health status and facilitates timely discharge of clients from the program.
About You- You should possess previous relevant case management experience in either a healthcare setting or in the community.
- You should have demonstrated ability to work both autonomously and in a multi-disciplinary team.
- You should have well-established assessment skills and the ability to identify psycho-social issues.
- You should have knowledge of and the ability to apply appropriate intervention strategies.
- You should have a good knowledge of community resources within the region.
- You should have a sound understanding of medical terminology.
- You should have effective interpersonal skills and the ability to communicate with a broad range of internal and external customers.
- You should be self-motivated with a demonstrated innovative approach to work practices and the ability to streamline and prioritize workload with flexibility in moving between tasks at short notice.
