Job Title: Case Manager
Job Summary: We are seeking an experienced Case Manager to join our team at Lifestyle Solutions. As a Case Manager, you will be responsible for providing excellent case management support to children and young people, families, and relevant stakeholders within our Intensive Therapeutic Care Program.
Key Responsibilities:- Provide case management support to children and young people with complex needs, including those with disabilities or complex trauma backgrounds.
- Work collaboratively with multidisciplinary teams and other services to ensure person-centered and strengths-based intervention and support.
- Develop and implement individualized case plans that address the unique needs and goals of each child or young person.
- Monitor and evaluate the effectiveness of case management interventions and make adjustments as needed.
- Maintain accurate and up-to-date records of case management activities and outcomes.
- Bachelor's degree in Social Work, Social Welfare, Psychology, Nursing, or Mental Health.
- Experience in case management in the community/social sector.
- Knowledge and demonstrated experience working within relevant legislation, OOHC Standards, policies, and procedures.
- Ability to apply culturally appropriate case management practices with Aboriginal and culturally diverse communities.
- Valid driver's license and Working with Children Check.
- Ability to work flexible hours and vaccination against Covid-19.
