We are seeking a skilled Therapeutic Case Manager to join our team at Lifestyle Solutions. As a Therapeutic Case Manager, you will provide excellent case management support to children and young people, families, and significant others within our Intensive Therapeutic Care Program.
The ideal candidate will have a Bachelor's degree in Social Work, Social Welfare, Psychology, Nursing, or Mental Health, and experience working with children and young people who have experienced complex trauma backgrounds. You will also have knowledge and demonstrated experience working within relevant legislation, OOHC Standards, policies, and procedures.
Key Responsibilities:
- Provide case management support to children and young people, families, and significant others
- Develop and implement individualized case plans to meet the needs of children and young people
- Collaborate with multidisciplinary teams to ensure comprehensive support
- Work with children and young people to identify and achieve their goals
