Job Title: Community Support Worker
We are seeking a dedicated and compassionate individual to join our team as a Community Support Worker. This rewarding role involves providing support to clients with disabilities, empowering them to live independently and participate fully in their communities.
The successful candidate will work closely with clients, families, and healthcare professionals to develop personalized care plans, promoting client autonomy and dignity. Key responsibilities include:
- Accompanying clients on social outings and activities, such as shopping trips or appointments;
- Providing assistance with daily living tasks, including toileting, showering, and dressing;
- Liaising with family members and representatives to ensure seamless communication and coordination of care;
To be successful in this role, you will possess excellent communication and interpersonal skills, as well as a strong understanding of the NDIS funding framework. A valid driver's license and reliable vehicle are also essential requirements.
