About Our Support Coordination Services

We are a dedicated organisation that aims to enhance the lives of individuals with disabilities through quality support coordination services. Our primary objective is to empower individuals to achieve greater independence, community participation, and personal wellbeing.

Key Responsibilities:
  • Assisting participants in building the skills necessary to understand and utilise their NDIS plan effectively.
  • Working with participants to ensure access to a diverse range of supports aimed at increasing their capacity to maintain relationships, enhance their ability to manage service delivery tasks, promoting greater independence, and facilitating community inclusion.
  • Building trust and rapport with participants, particularly those facing psychosocial challenges.
  • Collaborating with participants, families, carers, and other services to identify, plan, design, and coordinate tailored NDIS supports.
Requirements:
  • Tertiary qualifications in peer work or mental health, such as a Certificate IV in Mental Health Peer Work or Certificate IV in Mental Health, or equivalent training.
  • At least three years experience in the Support Coordinator / Psychosocial Recovery Coach role.
  • Experience in mental health-related work.
  • A valid NDIS Worker Screening Check and all relevant mandatory documentation.
