Key Responsibilities:
  • Provide tailored support to consumers and their families, facilitating capacity building and enhancing overall well-being.
  • Work collaboratively with the team and clients to achieve shared objectives.
  • Maintain accurate records and reports as required.
Requirements:
  • Hold a valid 'C' Class Driver's License and maintain a roadworthy vehicle.
  • Obtain a Working With Children's Check.
  • Secure National Police Clearance (within 6 months).
  • Possess First Aid Qualifications (HLTAID004 or equivalent).
  • Acquire COVID 19 Infection Control Certificate.
  • Meet NDIS Worker Screening Check requirements.
Job Overview:

The Equipped Centre provides person-centred allied health therapy, behaviour support and other services to people with disabilities. As a disability support worker, you will provide safe and individualised support to facilitate capacity building and enhance quality of life. Your role will change according to the needs of each client, identified through discussion between the individual, their family and your Team Leader.

Shifts will be primarily in North Perth and may include: Community access/leisure activities, General mentoring for young adults/teenagers, Support to implement routines/learn daily living skills, Medical or therapy appointments, Behavioural support.

Licenses and checks required: Valid 'C' Class Driver's License and roadworthy vehicle, Working With Children's Check, National Police Clearance (within 6 months), First Aid Qualifications (HLTAID004 or equivalent), COVID 19 Infection Control Certificate, NDIS Worker Screening Check.
