About the Role

This part-time opportunity is ideal for a skilled Clinical Care Coordinator to join our multidisciplinary team in delivering high-quality, evidence-based clinical care services. The successful candidate will contribute to enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention for young people up to the age of 25.

Key Responsibilities:
  • Deliver comprehensive, integrated, and person-centred care across the region.
  • Promote holistic wellbeing by considering broader determinants of health.
Requirements:
  • Must have one of the following: Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR, AASW registration/eligibility as a Social Worker, OR, Level 2 ACA registration/eligibility.
  • Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice.
  • Highly developed communication (including written) and interpersonal skills.
  • Experience in the use of electronic client/customer management systems and Microsoft Office applications.
  • Eligibility to apply for NDIS worker screening clearance.
