Job Opportunity

We are seeking a dedicated professional to join our multidisciplinary team delivering care coordination services for children and youth, focusing on early intervention and support of young people up to the age of 25.

The purpose of this role is to contribute high-quality evidence-based clinical care as a member of the consortia team. This involves addressing service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.

As a member of the team, you will promote holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.

  • Develop and implement individualized care plans in consultation with clients, families, and other stakeholders.
  • Provide education and training to staff, families, and carers on mental health issues and strategies.
  • Conduct assessments and develop interventions to address mental health and wellbeing needs of clients.

This is a part-time opportunity that has the flexibility to work 4 days per week or a 9 day fortnight. The role is primarily based at the hub in Gympie, with some flexibility to work from home one day per week.

Requirements
  • Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR AASW registration/eligibility as a Social Worker, OR Level 2 ACA registration/eligibility.
  • Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice as well as demonstrated excellence in the practical application of such, in the areas of mental health and alcohol and drug use.
  • Highly developed communication (including written) and interpersonal skills.
