Job Overview
We are seeking a dedicated professional to fill the position of Care Coordinator.
This role involves delivering culturally sensitive care to families with complex health needs, and working collaboratively with other services to achieve positive outcomes.
The ideal candidate will have strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
Key Responsibilities- Provide care coordination services to families with complex health needs, including developing and implementing individualized plans to meet their unique needs.
- Collaborate with families, healthcare providers, and other stakeholders to ensure comprehensive and coordinated care.
- Engage in ongoing assessment and evaluation of client needs, and make adjustments to care plans as necessary.
- Contribute to the development and maintenance of a trauma-informed and culturally safe environment.
- Maintain accurate and up-to-date records of client interactions and care activities.
- A degree in Social Work or a related field, and eligibility for membership with the Australian Association of Social Workers (AASW).
- A Working With Children Check (WWCC) and applicable vaccination requirements for a Category A role.
- A valid NSW driver's license.
