Job Description

As a Community Leader, you will oversee a multidisciplinary team to deliver high-quality care services to Aboriginal Home Health customers. You will inspire your team to work together in a community of health and care professionals who empower their clients to understand wellbeing needs and navigate healthcare systems.

Key Responsibilities
  • Promote collaboration among Care Team Leaders and neighbourhood teams to drive customer-focused results.
  • C Coordinate day-to-day operations to ensure safe, high-quality care delivery.
  • Develop strong relationships with external partners and agencies to enhance service integration and delivery.
Requirements
  • Hold a Certificate IV or Diploma in a relevant field or equivalent experience.
  • Bring operational experience in the care industry.
  • Lead geographically dispersed teams effectively.
