
Healthcare Coordinator Specialist

This is an exciting opportunity to play a key role in coordinating healthcare services and developing strategic partnerships across various health sectors to improve patient outcomes and experiences.
Key Responsibilities- Coordinate care and develop partnerships to enhance individual patient outcomes and experiences.
- Provide expert clinical advice to patients, carers, and other healthcare professionals within the district, community organisations, primary care and NGOs.
- Utilise a flexible and adaptive approach to patient care within a changing health system and support patients to navigate across the healthcare system.
The successful candidate will have the opportunity to work with a dynamic team to deliver high-quality patient care and contribute to the development of innovative healthcare solutions.
Selection Criteria- Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia.
- Minimum five years full-time equivalent postgraduate qualification experience and a postgraduate qualification in a relevant field of practice.
- Demonstrated experience working with and extensive knowledge of the broader health and social issues that people with chronic and complex conditions require.
